Hudson River HealthCare Receives Highest Level Recognition for Model of Care

The National Committee on Quality Assurance has awarded both HRHCare community health centers in Yonkers with the highest - Level 3 -  recognition under the NCQA Physician Practice Connections-Patient Centered Medical Home program.

NCQA’s program recognizes the work we do in providing quality care to our patients by rating us at the highest NCQA level that measures how well we provide access to appointments, management of patients with chronic disease, and ongoing quality improvement.
