Four HRHCare Health Centers Receive NCQA Level 3 PCMH Recognition

PCMHPeekskill, NY – HRHCare (Hudson River HealthCare), a nonprofit, Federally Qualified Health Center offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is proud to announce that the HRHCare Health Center at Hudson, the HRHCare Livingston Street Health Center at Poughkeepsie, the HRHCare Maxine S. Postal Tri-Community Health Center at Amityville and the HRHCare Kraus Family Health Center at Southampton have all received the National Committee for Quality Assurance (NCQA) Level 3 Patient-Centered Medical Home (PCMH) recognition. The highest PCMH ranking available, this recognition solidifies these four health centers, along with the 18 other HRHCare Health Centers that have already achieved Level 3 PCMH Recognition, as medical homes that emphasize care coordination and communication to transform primary care into what patients need and want it to be.

Saving the nation $24 billion annually and bringing $26.5 billion in economic activity to the communities they serve, 59% of all Community Health Centers have been recognized by the NCQA as Patient-Centered Medical Homes. This is directly tied to the Community Health Center model of affordable comprehensive care; integrating medical, dental and behavioral health care along with other services to facilitate health care access and improve health outcomes. HRHCare is proud to have 22 Health Centers that have achieved Level 3 PCMH status. Achieving this high level recognition depends on compliance with all of the must-pass PCMH elements which include data-proven standards for patient access and communication, implementation of evidence-based guidelines for chronic or other important conditions, active support of patient self-management, and systematic tracking of test results and clinical referrals.
