HRHCare Becomes Partner in Suffolk Care Collaborative

PrintPeekskill, NY – HRHCare (Hudson River HealthCare), a nonprofit, Federally Qualified Health Center offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is pleased to announce its inclusion as a partner in the Suffolk Care Collaborative, the Performing Provider System for Suffolk County under the Delivery Reforms Incentive Program (DSRIP). James Sinkoff, Executive Vice President and CFO of HRHCare, has also been appointed to serve on the newly formed panel. Led by Stony Brook Medicine, the Suffolk Care Collaborative has resulted from the recent partnership of thousands of health care delivery partners across Suffolk County, New York.

DSRIP is a federally funded statewide initiative that aims to significantly improve the way medical care is delivered to Medicaid and uninsured patients by establishing a large network of collaborating health care providers. As a Performing Provider System for DSRIP in Suffolk County, the goal of the Suffolk Care Collaborative is to meet the requirements of a Triple Aim Initiative; improving the patient experience of care, improving the health of populations and reducing the per capita cost of health care. As a result, a 25 percent reduction in avoidable hospital and emergency department use among Medicaid and uninsured populations is expected over a five-year period. If partnering organizations are successful in meeting the program’s strict merits, they will be able to share in an estimated $300 million in DSRIP reimbursements allocated for Suffolk County alone. Several of the key projects to be undertaken include asthma home-based self-management, substance abuse and other emotional-behavioral disorder prevention, and evidence-based strategies for diabetes and cardiovascular care.
