Coram, New York;  August 11, 2014 – HRHCare (Hudson River HealthCare) is pleased to announce that the Elsie Owens Health Center in Coram, Suffolk County,  has become HRHCare’s 17th Health Center to be recognized by the NCQA as a Level 3 Patient-Centered Medical Home.

The NCQA (National Committee for Quality Assurance) created the patient-centered medical home model of care delivery to provide personalized care and a long-term healing relationship between physician and patient. This goal is achieved through compliance with 10 must-pass elements including a high level of accessibility, a collective responsibility for each patient’s care among the health care team and taking full advantage of the latest health care information technology. As a Level 3 Patient-Centered Medical Home, the HRHCare Elsie Owens Health Center has achieved the highest level of compliance with the NCQA guidelines for excellence in providing patient-centered care.

In a recent statement on the Elsie Owens Health Center’s achievement of NCQA recognition, HRHCare President and CEO Anne Kauffman Nolon, MPH, said, “As is our long tradition, this has been accomplished by the hard work and collaboration of so many people who all recognize and embrace our most central core value: providing outstanding, compassionate care to the patients we serve. The Elsie Owens Health Center in Coram joined the HRHCare family in 2012 and serves over 7,400 patients with the highest quality medical, dental and behavioral health care.
