HRHCare Patient Navigator Wins Planetree Award for Innovation in Patient Care

Peekskill, NY – HRHCare (Hudson River HealthCare, Inc.), a nonprofit, Federally Qualified Health Center offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is proud to announce that Patient Navigator Sabrina Rose will be presented with a Planetree Award for Innovation in Patient Care. This award is presented by the HRHCare Planetree Institute to exemplary employees who find new ways to advocate for patients and create innovative ways for them to access high quality and affordable health care.

A Patient Navigator at the HRHCare Atrium Health Center at Poughkeepsie, Sabrina Rose has been a part of the HRHCare family since May of 2014. With 14 years spent in the food and beverage service industry and first-hand experience in Cancer Services at all three of HRHCare’s Health Centers in Poughkeepsie, it was no surprise that Sabrina quickly took the lead in delivering passionate patient-centered service, especially when it came to stressing the importance of cancer screenings. Sabrina consistently went out of her way to make personal calls to numerous female patients encouraging them to have breast cancer screenings done. As an added incentive, she put together HRHCare goodie bags filled with breast cancer awareness materials, self-examination cards and pink carnations which she handed out to those women who came in for a mammogram. This personal touch not only calmed the fears many people have surrounding cancer screenings but has also helped to save lives. When asked about her outlook on today’s health care industry, Sabrina replied, “Most people in the health care industry are in it because they care.  We bend over backwards for our patients and will always try to do whatever we can for their best interest.” When not delivering the incredible health care for which she’s known, Sabrina continues to tend bar as well as kayak, try new food and “occasionally dabble in karaoke.”

“Sabrina has gone above and beyond to help and encourage women to get needed cancer screenings done,” said Soraya McMahon, Patient Care Partner at the HRHCare Atrium Health Center at Poughkeepsie. “Her outreach has made a great impact on our Health Center quality reports, but more importantly on the lives of so many women.”

The HRHCare Planetree Innovation Award honors employees who have been nominated by their colleagues for creating new initiatives and programs that improve the lives of patients and the care they receive.
