Peekskill, NY – HRHCare (Hudson River HealthCare), a nonprofit, Federally Qualified Health Center offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is proud to announce that Elizabeth Feliciano, an outreach worker based at the HRHCare Maxine S. Postal Tri-Community Health Center at Amityville, will be presented with a Planetree Spirit Award for excellence in patient care. HRHCare’s Planetree Institute presents these awards to employees who go above and beyond to make health care more personalized and create a warm and welcoming environment for all.

Joining the HRHCare family six months ago, Elizabeth Feliciano, mother of four and grandmother of ten, has brought her valuable experience with the Urban League of Westchester to Long Island; embracing the challenge of presenting HRHCare’s full complement of services to vulnerable populations in a new area with excitement and passion. As an outreach worker, medical interpreter and New York State Department of Health certified assistor, Elizabeth is a sure presence at nearly every HRHCare outreach event on Long Island and has consistently been praised for her warmth by staff and patients alike. She has been a great supporter of HRHCare’s outreach efforts to the LGBT community in Long Island and has recently helped to organize an HRHCare sponsored Health Fair at the Amityville Public Library; giving attendees the opportunity to learn about SNAP, veterans services, health insurance enrollment, nutrition and to receive various health screenings. “I am so happy that a patient can come to HRHCare and get all kinds of help, even beyond health care,” said Elizabeth. “We have totally changed the perception of what a health center is.”

“Elizabeth is an inspiration,” said Kathy Brieger, Executive Director of the HRHCare Planetree Institute. “Her concern for our patients is so evident, always seeking clarification when questions come up or when the patient is delayed in receiving care. She is patient-centered and consistently goes beyond her normal duties with great compassion.”

Planetree is the global leader in advancing patient-centered care solutions, integrating the technology of modern medicine with the healing dimensions of compassion, comfortable surroundings, shared knowledge and freedom to make educated choices. HRHCare has been an affiliate since 1996 and remains the only Federally Qualified Health Center to be a part of this alliance. The HRHCare Planetree Institute was established in 2012 and provides state-of-the-art training to prepare staff in delivering passionate and personalized care. The HRHCare Planetree Spirit Award honors employees who have been nominated by their colleagues for going above and beyond to provide individualized patient care and enhance health care services in their respective facility.
