LIHC Awarded the Population Health Improvement Program (PHIP) Contract

2350NSHC LogoHRHCare is pleased to announce that the Nassau-Suffolk Hospital Council, an extension of the Long Island Health Collaborative in which HRHCare is a participant, was awarded the Population Health Improvement Program (PHIP) contract for the Long Island region by the New York State Department of Health; a total award of $2.4 million for a period of two years. The goal of the PHIP is to promote the “Triple Aim” of better care, better population health and lower health care costs. The PHIP contractors will convene meetings of local stakeholders and provide a neutral forum for identifying, sharing, disseminating and helping to implement the best practices and strategies to promote better health outcomes and reduce disparities across the population in each region.

The Long Island Health Collaborative (LIHC) is a working group of hospital members, local health department personnel, representatives from social service organizations, public health specialists from colleges and others who form the core of health and human services for all Long Islanders. LIHC’s goal is to help Long Islanders understand why prevention and primary care are important, why each patient must play a more active role in their own health, what resources and services are available and how they can be accessed.
