Peekskill, NY – HRHCare (Hudson River HealthCare), a nonprofit, federally qualified healthcare system offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is proud to announce that Family Nurse Practitioner Hyerim Gabriel will be presented with a Planetree Award for excellence in patient care. HRHCare’s Planetree Institute presents these awards to employees who go above and beyond to make healthcare more personalized and create a warm and welcoming environment for all.

Hyerim Gabriel, known by her patients and fellow staff as Clara, has touched the lives of countless patients at HRHCare’s health center on Valentine Lane in Yonkers over the past five years. Known for her strong work ethic, attention to detail and love of people, she truly embodies what both the Planetree Institute and HRHCare are all about. Hyerim received her BSN in Nursing from EWHA Women’s University in Seoul, Korea, her FNP from Columbia University School of Nursing, and is a Certified Family Practitioner with the American Nurses Credentialing Center.

“Clara (Hyerim) is well known throughout HRHCare for her compassion and energy,” says Yonkers Patient Care Representative Petramaris Pena. “She emulates warmth, humility and consideration as she serves others and always expresses appreciation for others and their efforts. She is highly respected by her peers and appreciated by her patients.”

Planetree is the global leader in advancing patient-centered care solutions, integrating the technology of modern medicine with the healing dimensions of compassion, comfortable surroundings, shared knowledge and freedom to make educated choices. HRHCare has been an affiliate since 1996 and remains the only Federally Qualified Health Center to be a part of this alliance. The HRHCare Planetree Institute was established in 2012 and provides state-of-the-art training to prepare staff in delivering passionate and personalized care. The HRHCare Planetree Award honors employees who have been nominated by their colleagues for going above and beyond to provide individualized patient care and enhance healthcare services in their respective facility.
