HRHCare and Sharon Hospital Offer Free Chronic Disease Self-Management Course

Sharon HospitalPeekskill, NY – HRHCare (Hudson River HealthCare), a nonprofit, Federally Qualified Health Center offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is partnering with Sharon Hospital to offer a free six-week Healthy Choices NY workshop series on chronic disease self-management beginning June 12th in Sharon, Connecticut. This workshop is open to all adults and their caregivers dealing with any ongoing health condition including diabetes, pain, arthritis, memory loss, depression and high blood pressure. The two and a half hour group discussions will include topics such as use of medication, discovering better food and exercise choices and finding practical ways to deal with pain, fatigue and stress. Participants will have the opportunity to set their own goals and make step-by-step action plans to improve their health and lifestyle.

The free chronic disease self-management workshops will be held each Friday from 10:30 AM to 1:00 PM at Sharon Hospital, 50 Hospital Hill Road, Sharon, Connecticut. The dates are June 12th, 19th and 26th and July 3rd, 10th and 17th.
